The card promised ₹5 lakh. But which hospital? Which code?

Health schemes promise coverage but hide the grid underneath—empanelment lists, procedure codes, hospital capabilities. A ₹5 lakh promise means nothing if the family doesn't know which hospital qualifies or which code matches their procedure. They have a card but no map.
